An author crashes his car in a snowstorm, and awakens to find he's not just heavily injured, but taken in by a nurse who claims to be the number one fan of his books. During his stay with her, she gets the man addicted to painkillers, and lordes them over his head so that he'll share the manuscript with the next book in his series... and she disapproves, forcing him to rewrite it.
But the important thing is - at some point he manages to escape, but is caught once more. To keep him from being able to escape again, she destroys his legs with mallets so that he'd be unable to walk and be forced rely on her forever.
Though, if I recall correctly, she used an axe on them in the book proper... but as this was a film adaption, there was little they could do about their main actor needing to keep his foot.
